Answer: 要介護高齢者を日中6～8時間ほど預かるデイ・サービス。こうした施設でそのまま高齢者が泊れるようにしようという仕組みが宿泊付きデイ・サービスだ。保険除外サービスとして既に多くの事業者が提供している。
日本介護福祉グループ（東京都墨田区）が全国324か所でフランチャイズ形式のデイサービスを実施している「茶話本舗」もその一つ。民家を活用している利点を生かし、和室に布団を敷いたり、仮説ベッドを使ったりして宿泊を受け入れている。

宿泊サービスがあるから在宅介護が続けられる
東京都のB子さん（64）は、兄（82）を隔週で茶話本舗に宿泊させている。兄は介護度が5で妻（84）と二人暮らし。妻も高齢で体調がすぐれず、近くに住むB子さんが毎日介護を手伝っている。
「二人がかりでも日々の介護は大変。でも兄の希望もあり、施設には入れたくない。宿泊サービスを使って、時々休息をとれるから義姉と私も在宅介護が続けられる」
専門家は導入に慎重
デイサービスは全国に約2万6千か所もあり、身近な場所で宿泊を受け入れてもらえるようになれば家族も助かる。厚労省も宿泊付きデイサービスの制度化は、在宅で介護している家族の負担軽減つながるとみている。ただ導入には専門家の反対も強い。
一時的な宿泊なら現在の介護保険にもショートステイというサービスがある。新たなメニューを増やすよりショートステイの普及に力を入れるべきだという意見だ。宿泊を前提としないデイサービスの施設で高齢者を泊らせる安全上の問題を指摘する声もある。
認知症高齢者には特に有効
国に先駆けて2009年度からデイサービスでの宿泊を試行する東京都は、利用対象者を認知症に限定している。事業委託を受けている特別非営利活動法人（NPO法人）サポートハウス年輪（東京都西東京市）の安岡厚子理事長は「認知症の高齢者は、環境の変化に弱い。なれない施設のショートステイを利用して症状が悪化したり、本人が嫌がって途中で帰されたりする場合もある」と説明する。
サポートハウス年輪では、昨年10月に宿泊を始めて以来、平均して週1～2件の利用があり、この間こうした問題は起こっていない。「通いなれた場所、見知ったスタッフだから認知症の高齢者も落ち着いて泊れる。家族の宿泊希望は強い。結論を急がず、制度化に向けて実施を検証してほしい」と安岡さんは話す。
